What to expect when seeing Dr. Hazen for the first time -

 

 

As many of you know, your favorite chiropractor Lee Hazen D.C. has combined forces with Erik Lundquist M.D. to form what we affectionately call the Temecula Center for Integrative Medicine (TCIM).  A lot of blood, sweat, and tears have gone into developing this clinic and we are so excited to share it with you.  In addition to chiropractic and massage therapy which you are used to, you now have access to additional healthcare related services such as medical care, naturopathy, acupuncture, and nutritional counseling.  The best part is…it’s all under one roof!

If you have been a long time patient of Dr. Hazen there may be a few changes that you will experience the first time you come in to see him.  The purpose of this e-mail is to highlight these changes so you know what to expect and can plan accordingly. 
 

Insurance

 

One of the greatest benefits of Dr. Hazen and Dr. Lundquist working together is that now, with most insurance carriers, we will be able to cover all of your chiropractic treatments AND any other medically necessary services like integrative and functional medical care (ask us what this is if you don't know what it is), primary care, naturopathic consultation, massage therapy, and more.  What does that mean?

 

You can access your insurance benefits for both chiropractic treatments and additional ancillary medical treatments because we are located in the same facility. This is unique to TCIM based on the partnerships we have created and our integrative holistic approach to medicine.  We hope that this is great news for you giving you more access to healthcare alternatives by using your insurance plan.

Medical Evaluation

 

During your first visit with Dr. Hazen at TCIM, one of our amazing medical providers will meet with you for approximately 10 minutes (along side Dr. Hazen) to perform a quick medical evaluation. 

This evaluation opens the door to all of the amazing resources available to you at TCIM.  The medical provider acts as the gatekeeper to the services at TCIM and indicates to the insurance company that you have a reason to be evaluated and treated by a chiropractor. This combination of a medical provider with Dr Hazen allows you to be part of a truly “integrative” healthcare delivery system.

This medical evaluation is only necessary as part of your initial visit with Dr. Hazen at our clinic for that particular problem.  Your typical office visit co-payment will be charged.  We hope that you see the benefit of this brief exam as something that will enhance the quality of your overall healthcare experience. 

We are really excited for you to see the unique partnership between Dr. Hazen and the medical providers here at TCIM and begin to experience the total benefit of Integrative Medicine. 

Paperwork

 

We know that nobody likes to fill out paperwork.  But unfortunately, you are going to have to fill out “new patient” paperwork during your first visit to TCIM.  We know…we feel the same way.  Say it loudly with us…Boooo!! 

We really wish we could magically skip this step but even though you may be a long-time patient of Dr. Hazen’s, you are technically a “new patient” to TCIM and we need you to fill out new updated paperwork.  It is a compliance issue that we cannot avoid.

 

We will try to make this process as easy as possible and we thank you in advance for understanding the necessity of this step.  

 

Please allow 15-30 minutes of additional time for all of these processes to take place during your first visit.

Dr. Hazen's New Office Hours

Dr. Hazen's office hours are below.  He is still accepting walk-ins...but it is extremely helpful if you can call the office first to let us know you are coming in.  It really helps our team with scheduling and work flow and will help other patients with wait times.

Dr. Hazen's new schedule will allow him to see more patients and hopefully give you the opportunity to make appointments that fit your schedule.

Dr Hazen is now seeing patients on FRIDAY AFTERNOONS, note the updated schedule below.

Monday - Friday

8:00 a.m. - 12:00 p.m. and from 2:00 p.m. - 5:00 p.m.
